Corporal Charles W. Cook of Captain Horace Nile's
Company (E), of the Thirty-fifth Regiment of Massachusetts
Volunteers, was enlisted by the Selectmen of Milton of the
Regiment of , at Milton
on the Second day of August 1862, to serve three
years; he was born in Milton, in the State of Massachusetts
is twenty two years of age, six feet one half inches high,
light complexion, hazel eyes, light hair, and by occupation
when enlisted Farmer
During the last two months said soldier has been unfit for duty 61 days.

I certify that I have carefully examined the said Charles W. Cook
of Captain Horace Nile's Company, and find him incapable of performing
the duties of a soldier, because of

a wound of the ankle joint, with a loss
of bone, some recovered portions of which still
remain adherent. The full use of the joint
cannot be recovered in months if ever.
